Bridging Health and Social Care: PCSI and Highmark Partner for Healthier Futures

Latoya Johnson presents to Highmark Executive Team members

As a proud member of Highmark Health ‘s Social Care Network since 2022, PCSI continues to champion the well-being of individuals referred through Highmark’s online community support platform.

On June 3, 2025, Latoya Johnson, PCSI Case Management Coordinator, and Sarah Cook, PCSI Executive Director, had the honor of presenting to Highmark Health’s Executive Team. In collaboration with Allegheny Health Network’s Center for Inclusion Health and its Healthy Food Center, PCSI walked the team through a case study of a hypothetical patient facing complex medical and social challenges. The presentation showcased the breadth and depth of PCSI’s services, emphasizing how these offerings can significantly improve outcomes for Highmark-referred patients by addressing social drivers of health.

From emergency assistance to long-term self-sufficiency coaching, PCSI provides a comprehensive suite of resources designed to help individuals and families achieve stability and upward mobility. “Our partnership with Highmark and AHN allows us to meet people where they are and connect them with the support they need to thrive,” said Sarah Cook, Executive Director. “For PCSI, as Pittsburgh’s Community Action Agency, it’s always been about more than just services; it’s about creating pathways to long-term financial, social, and physical well-being.”
