- Your source(s) and amount(s) of income
- A list of all members in your household and their sources and amount of income
Depending on the services you’re seeking, we may also ask for:
- Your utility bill(s) and any shutoff notice you have received
- Your landlord’s name and contact information
- Your lease or rental agreement and an eviction notice if you’ve received one
- Your resume, if you have one
- Documents establishing your employment authorization
We will collect additional information that helps us to tailor our services and get to know you and your needs better (e.g., veteran status, type of health insurance, etc.)
